Westport Man Accused of Forging Prescription at Local CVS

Westport police reports: March 8-12.

Bradley H. Jack, 53 of, 290 North Avenue, Westport, was charged Friday with second-degree forgery after he turned himself into police Westport Police Department on a warrant stemming from an incident in November 2011.

At that time, Jack allegedly forged the date of a prescription for a controlled substance at which was made out to him by a doctor from Fairfield, police said. He posted $5,000 bond and was released for court March 21.

Jack was accused of a similar crime in June, 2011 where he allegedly tried to obtain Oxycontin and Ritalin from CVS.

Bradley is reportedly owner of the most expensive residential property in Fairfield  — a 20-acre waterfront estate on Sasco Hill Road in Fairfield, which he owns with his ex-wife, Karin Jack. The two are reportedly separated and Jack currently lives in Westport.

The Jacks' house on Sasco Hill Road was assessed in Fairfield’s recent town-wide revaluation at $24.2 million, an increase of $1 million over the prior assessment of $23.2 million. The new assessment gives the Jacks' house a market value of $34.6 million.
